Hospital Passport

This is a document which parents can fill in and include details about your child such as how your child communicates, information about their likes and dislikes as well as details such as background medical information.

If you and your child need to go to hospital you can hand this document to the hospital staff and do not need to explain/verbally provide information at a time when you and your child are already in a stressful situation.
